Returning Student
Collect Key documents needed for submission of the application.
If you do not have all the documents for the application, you are able to return to your application.
US Domestic Students:
On Campus
- Insurance Information (if none, student must sign a waiver)
- Immunization records (childhood and adolescent immunization - not CoVid shots)
- High School Diploma / GED
- High School Transcript(s) – Use the application request process. English translation and evaluation may be required.
- ACT/SAT/Accuplacer official scores and documentation (Test takers name and date must be visible)
- Any College transcripts – Indicate the number of institutions and use the application request process. English translation and evaluation may be required. The sending institution will transmit the transcripts directly to admissions@champion.edu
- Two references – provide email address for two references (non-family or guardian)
Online
- High School Diploma or G.E.D.
- High School Transcript(s) – Use the application request process. English translation and evaluation may be required. The sending institution will transmit the transcripts directly to admissions@champion.edu
- ACT/SAT/Accuplacer official scores and documentation (Test takers name and date must be visible)

International Students:
(Submit 3 months prior to the semester start date in which the student wishes to enroll, unless the student is transferring from an institution already in the United States. Students transferring to Champion will need to complete the Transfer Eligibility Form.)

- Visa Status
- I-20 Status
- Passport # and Picture of the Front Page with all critical information
- Financial Info – current copy of savings or checking account illustrating ability to pay
- Financial Support Affidavit - signed by student and any financial supporter(s)
- English Proficiency: Test of English as a Foreign Language (TOEFL) score of 70 (iBT), International English Language Testing System (IELTS) score of 6 or higher, Successful completion of High Scholl in which instruction was in English, completion of high level ESL courses.
